Post Acute Transition Specialist

CarePartners is seeking a skilled Post Acute Transition Specialist to join our team. This role will act as a coordinator between internal and external referral sources and our Post-Acute Service Lines.
Key Responsibilities- Establish relationships with hospitals, skilled nursing facilities, assisted living facilities, ambulatory and inpatient care managers, physicians, payors, and other community referral sources.
- Provide education and promote collaboration to ensure appropriate patient placement.
- Work closely with Post-Acute providers, SNFs, and physician practices to ensure a smooth referral and admission process.
- Collaborate with health system and community colleagues to ensure quality-driven, best practice across the continuum.
- Maximize positive financial outcomes for designated patient populations by facilitating appropriate Post-Acute placement and participating in Goals of Care/Advanced Care Planning dialogue.
- Bachelor of Science in Nursing or related field.
- Minimum of two years recent experience in a health-related field.
- Preferred experience in clinical care coordination or a related field.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
