Job Summary

We are seeking a highly skilled and compassionate Admissions Intake Coordinator to join our team at Heart to Heart Hospice. As a key member of our admissions team, you will be responsible for ensuring seamless patient referrals and intake operations, while maintaining compliance with regulatory requirements and building strong relationships with referral sources.

Key Responsibilities
  • Direct daily patient referral and intake operations, including establishing and implementing intake policies
  • Initiate patient charts into our electronic medical record (EMR) system and maintain workflow
  • Maintain census, pending admissions, red flag, journey's end, and care plan meetings on our stand-up board
  • Ensure compliance with state, federal, and CHAP regulatory requirements related to referrals and intake
  • Establish and maintain positive working relationships with current and potential referral sources, building and monitoring community and customer perceptions of Heart to Heart Hospice as a high-quality provider of services
  • Gather, collate, and report referral statistics, including key customer referral trends
  • Participate in strategic planning, identifying opportunities for additional or improved services to meet unmet customer needs
  • Maintain comprehensive working knowledge of Heart to Heart contractual relationships and ensure patients are admitted according to contract provisions
  • Participate in quality assessment and performance improvement teams and activities
  • Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources when services are not provided by Heart to Heart Hospice
  • Timely submission of all documentation
  • Perform other duties as required
Requirements
  • 3-5 years of experience in healthcare, preferably in hospice operations
  • Admissions/intake and customer service experience
  • Valid driver's license and automobile insurance
  • Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices
  • Experience with EMR software
