Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Hospice Team in Mishawaka. As an LPN, you will play a vital role in providing high-quality care to patients and their families, working collaboratively with our interdisciplinary team to ensure seamless care coordination.

Responsibilities
  • Administer Skilled Nursing Care: Under the supervision of a registered nurse, you will provide skilled nursing care to patients in their place of residence, ensuring their physical, psycho-social, and environmental needs are met.
  • Coordinate Care: You will work closely with our interdisciplinary team to coordinate care, ensuring patients receive comprehensive and coordinated care.
  • Communicate Effectively: You will provide effective communication to patients, families, and healthcare professionals, ensuring seamless care coordination and patient satisfaction.
  • Monitor Cases: You will monitor assigned cases to ensure compliance with requirements of third-party payors, ensuring timely and accurate reimbursement.
Requirements
  • Education: Graduate of an accredited school of vocational nursing.
  • Experience: One year experience as a Licensed Vocational Nurse in a clinical care setting required; home health experience preferred.
  • Knowledge and Skills: Nursing skills as defined by generally accepted standards of practice; good interpersonal skills.
  • Transportation: Reliable transportation is necessary, as this position may require occasional travel to alternate locations.
