Job Overview: Bridgewell is seeking a Support Professional 2 to assist individuals in Level 2 residential programs. The successful candidate will provide daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community.

Key Responsibilities:
  1. Provide support and assistance with skills training in all activities of daily living.
  2. Create opportunities for individuals to develop their skills and foster independence.
  3. Ensure services are provided according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, as well as Bridgewell policies and procedures.
  4. Support individuals with medical, clinical, and financial needs.
  5. Transport and accompany individuals as needed to and from medical, recreational, and other activities outside the program.
  6. Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.
  7. Access resources and recreational opportunities in the community.
  8. Administer medications according to MAP regulations, as applicable.
  9. Maintain core training certifications including FA/CPR, PABC, and MAP.
  10. Develop positive relationships with guardians, family members, and other stakeholders.


Requirements:
  1. A high school diploma or GED is required. An Associate's degree or DSP Certification is preferred.
  2. A minimum of one year of experience working with people with developmental or psychiatric disabilities is preferred.
  3. A valid driver's license and one year of driving experience are required.
  4. Excellent written and oral communication skills, as well as organizational and problem-solving skills, are necessary.
