Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at BrightSpring Health Services. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals with intellectual, developmental, or cognitive disabilities.

Responsibilities

  • Provide individualized care to clients, including vital sign monitoring, medication administration, and wound care.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments of clients' medical and social needs, and communicate findings to the care team.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal client outcomes.
  • Document client progress and communicate with family members, case managers, and physicians as needed.
  • Ensure accurate and timely completion of incident reports and other required documentation.
  • Participate in ongoing education and training to maintain current licensure and stay up-to-date on best practices in patient care.

Requirements

  • Current LPN licensure in good standing from the state(s) of practice.
  • Minimum 1 year of experience in a healthcare setting, preferably in a skilled nursing or home health care environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to lift 25 pounds and stand for long periods, with frequent bending and stooping.
  • Valid driver's license and reliable transportation.
