Job Overview

We are seeking a skilled Clinical Care Specialist to join our team. As a member of our clinical staff, you will play a vital role in providing direct patient care and supporting our multidisciplinary team. This is a flexible, per diem position that offers a competitive salary and opportunities for professional growth and development.

The ideal candidate will have experience working in hospice or home health settings, with a strong focus on patient-centered care and a commitment to delivering exceptional service. You will work closely with our registered nurses and other healthcare professionals to develop and implement comprehensive care plans tailored to each patient's unique needs.

Earn an estimated $55,000 - $65,000 annually

Key Responsibilities:

  • Provide direct patient care under the supervision of a registered nurse
  • Conduct regular assessments and update care plans as necessary
  • Collaborate with healthcare teams to ensure seamless transitions of care
  • Communicate effectively with patients, families, and caregivers
  • Participate in quality improvement initiatives and contribute to a culture of excellence

Requirements:

  • Certified Nursing Assistant (CNA) license
  • 6 months of patient care experience in hospice or home health settings
  • Current CPR certification
  • Valid driver's license and automobile insurance
