We are currently seeking a dedicated Hospice Registered Nurse to provide compassionate care in various settings. This role involves visiting multiple patients daily, typically within a 15-mile radius, during standard business hours from Monday to Friday.
Key Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Conduct focused interviews to identify specific patient needs.
- Monitor and assess signs and symptoms indicating physiological and psychosocial changes in patients.
- Collect, analyze, and interpret data from healthcare team members, documenting nursing diagnoses.
- Develop and document the patient's plan of care based on identified nursing diagnoses and expected outcomes.
- Prioritize actions based on patient care requirements.
- Implement interventions according to established priorities and care plans.
- Ensure a safe and therapeutic environment for patients.
- Provide education and support to patients and their families.
- Revise care plans based on evaluations and changes in patient conditions.
- Document patient responses to nursing interventions.
- Exercise clinical judgment in meeting patient care needs.
- Monitor the safety and effectiveness of the care environment.
- Support grieving family members with coping techniques and resources.
- Perform additional duties as assigned.
Minimum Qualifications:
- Active Registered Nurse license in the state of practice.
- Adherence to professional standards of practice.
- Completion of relevant competency programs as required.
- Current CPR certification.
- Compliance with health regulations (TB questionnaire, health certificate, etc.).
- Understanding of patient confidentiality and HIPAA regulations.
- Strong communication skills and proficiency in English.
- Proficient in computer usage.
- Must be at least 18 years of age.
