  • You will be responsible for providing comprehensive case management services to patients in their homes, ensuring seamless transitions between healthcare settings.

What You'll Need to Succeed
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One year of experience as a RN, preferably in home health or a related field.
