Job Title: Medical Provider

Job Summary:

We are seeking a highly skilled Medical Provider to join our team at Community Health Center, Inc. (CHC). As a Medical Provider, you will play a vital role in providing high-quality medical services to our patients, particularly those from underserved populations.

Key Responsibilities:

  • Provide a full scope of medical services, including routine check-ups, diagnoses, and treatment plans.
  • Work collaboratively with our multidisciplinary team to deliver individual and population-based care.
  • Participate in practice-based research and quality improvement initiatives to enhance patient outcomes.
  • Develop and maintain strong relationships with patients, families, and community stakeholders.

Requirements:

  • MD/DO from an approved medical/osteopathy school and graduation from an accredited residency program in family practice.
  • Board certification within 24 months of commencing employment at CHC and licensure or eligibility for licensure in the State of Connecticut.
  • Bi-lingual skills in English/Spanish preferred.
