We are seeking a highly skilled Patient Care Manager to join our team at Three Rivers Home Health Services. This role is ideal for a Registered Nurse looking for a challenging and rewarding opportunity.

The estimated annual salary for this position is approximately $**$85,000 - $110,000** depending on experience and location.

About the Role:

Job Description:

  • Provide clinical services within the scope of practice as defined by state laws governing nursing practice in accordance with the plan of care and in coordination with other healthcare professionals.
  • Coordinate patient referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Determine patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Evaluate patient needs and provide individualized care that meets home health eligibility criteria and is in accordance with physician orders.
  • Review assessments and plans of care daily and consult with clinicians as necessary.

Education & Experience:

  • Current RN licensure in the state of practice required.
  • Current CPR certification required.
  • Valid driver's license, vehicle insurance, and access to reliable transportation or public transportation.
