Role Overview:
The Primary Care Physician (PCP) serves as a leader within our collaborative care environment. We are committed to providing quality care to our patients.

Our care team includes a diverse range of professionals: Doctors, Advanced Practice Providers, Pharmacists, Care Coach Nurses, Medical Assistants, Behavioral Health Specialists, Quality Coders, Referral Coordinators, and more. This comprehensive approach enables us to deliver an exceptional experience for seniors.

Our model is designed to enhance care quality and improve outcomes for seniors by offering a personalized experience, multidisciplinary services, coordinated care supported by analytics, and strong community connections. This extensive support allows our PCPs to manage fewer patients while dedicating more time to each individual.

Key Responsibilities:

  • Assess and treat patients in accordance with established care standards.
  • Monitor patient care quality using available data and chart reviews.
  • Coordinate patient services, including specialty referrals and home health care.
  • Actively participate in care team discussions and daily huddles.
  • Foster a cooperative practice environment by maintaining a professional demeanor.
  • Complete medical documentation promptly, collaborating with quality coders to enhance coding accuracy.
  • Adhere to policies and protocols set by Clinical Leadership.
  • Engage with leadership regarding care quality and policy reviews.
  • Explore growth opportunities for new or existing services.
  • Participate in the local primary care on-call program as necessary.
  • Ensure compliance with licensing and accreditation requirements.
  • Focus entirely on direct patient care, including patient interactions and necessary administrative tasks.

Required Qualifications:

  • Current and unrestricted medical license or willingness to obtain one in the state of practice.
  • Graduate of an accredited MD or DO program.
  • Strong verbal and written communication skills.
  • Exceptional interpersonal skills and ability to communicate effectively with colleagues and patients.
  • Fully engaged in the integrated team-based care model.
  • Willingness to adapt to a value-based care setting.
  • Outstanding patient service skills.
  • Basic computer proficiency, including email and electronic medical records.
  • This role involves patient interaction and requires participation in a Tuberculosis (TB) screening program.

Preferred Qualifications:

  • Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Numbers.
  • Two to five years of relevant experience preferred.
  • Experience managing a Medicare Advantage patient panel.
  • Knowledge of Medicare guidelines and coverage.
  • Bilingual candidates are a plus.
  • Familiarity with HEDIS quality indicators.
