Job Summary:We are seeking a skilled Staff Nurse - Behavioral Health Specialist to join our team. The successful candidate will provide nursing care to assigned patients through assessment of patients' needs, making nursing diagnoses, developing and implementing a plan of care, which reflects an individualized mode of nursing intervention.
Responsibilities:- Provide nursing care to assigned patients through assessment of patients' needs, making nursing diagnoses, developing and implementing a plan of care.
- Initiate a nursing assessment and nursing care plan upon admission, which reflects long and short-range goals, patient teaching, and a discharge plan.
- Participate in the multidisciplinary treatment plan implementation.
- Collaborate with the head nurse in assigning functions to ancillary personnel.
- Record pertinent observations on patient charts and report verbally to the head nurse/team leader and/or physician.
- Evaluate the results of nursing intervention and make appropriate changes when necessary.
- Administer medication as ordered and chart same according to hospital policy and professional standards.
- Count narcotics with in-coming and out-going tour nurses according to hospital policy/procedures.
- Check code cart/emergency equipment as assigned.
- Perform control code daily and maintain the cleanliness of the glucometer.
- Counsel and/or instruct patients and/or family with evidence that the patient and/or family understand the instructions given.
- Participate in department and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice.
- Participate in ongoing PI activities and assist with data collection.
- Maintain professional competence through participation in continuing education, in-service staff development, and self-defectiveness in his/her own education process.
- Assist in coordinating and discharge planning monitoring activity for clinical services. Assist in providing ongoing technical assistance to interdisciplinary patient care teams and initiate the interdisciplinary discharge planning progress record for each patient. Complete referrals to home care and other agencies if required.
- Conduct medication groups, health education groups, and co-lead community activities.
- Function effectively during Code Blue and other medical emergencies.
- Carry out physician's orders concerning therapy and medications.
- Assist in the supervision of non-professional staff.
- Ensure a safe and secure environment free of hazards through ongoing environmental surveillance processes.
- Explain, involve, and instruct patients/families concerning medication and treatment modalities.
- Utilize the electronic as well as the paper medical record while maintaining patient confidentiality.
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
