Job Summary:

We are looking for a skilled and experienced Patient Care Specialist to provide high-quality patient care and support services to our patients. The ideal candidate will have strong communication skills, be able to work effectively in a fast-paced environment, and possess a deep understanding of patient-centered care.

Key Responsibilities:

  • Provide direct patient care and support services to meet the physical, emotional, and social needs of patients.
  • Develop and implement individualized patient care plans that address specific needs and goals.
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Conduct thorough assessments to identify patient needs and develop strategies for addressing those needs.
  • Maintain accurate and detailed records of patient interactions and progress.

Requirements:

  • A minimum of 2 years of experience as a Registered Nurse or equivalent.
  • Current RN licensure in the state where assigned.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Excellent problem-solving and critical thinking skills.

Salary: $62,000 - $82,000 per year, depending on experience.
