Join Our Team as a Wellness Mentor/Health Coach

Sierra Pacific Orthopedics, a leading full-service orthopedic group in the San Joaquin Valley, is seeking a full-time Wellness Mentor/Health Coach to join our Lean MD Practice.

Job Summary

As a Wellness Mentor/Health Coach, you will perform administrative and clinical duties in coordination with our physicians to provide quality healthcare to patients. Your primary responsibility will be to work one-on-one with patients to help them achieve their health and wellness goals.

Responsibilities
  • Room patients and take vital signs
  • Document patient information in our Electronic Medical Records (EMR) system
  • Provide step-by-step counseling to patients to keep them motivated and engaged in their weight loss journey
  • Conduct weekly consultations, make care calls, and live confirmation calls to ensure patient success
  • Mentor patients in identifying challenges, setting realistic goals, and achieving life-changing habits
Requirements
  • Coursework in health, medical nutrition, weight loss, and/or physical fitness
  • Medical Assistant Certification is a plus
  • Strong communication skills with staff, physicians, and patients
  • Good analytical and problem-solving skills
  • Ability to prioritize work flow in a fast-paced team environment
  • Strong computer skills, with experience in Electronic Medical Records (EMR) systems, preferably with Nextgen
  • Excellent social media skills with the ability to generate new ideas on how to connect with patients through various social outlets
Working Conditions

This position requires prolonged sitting, some bending, stooping, and stretching. Some patient assistance will be required. Good hand-eye coordination and manual dexterity are necessary to operate a keyboard, photocopier, telephone, and other office equipment. The employee must have normal range of hearing and eyesight to record, prepare, and communicate appropriate reports.
