Position Title: Outreach Care Specialist

Location:
This role is primarily field-based, requiring candidates to engage with the community directly. Candidates must reside in Connecticut to be eligible.

Target Areas:
We are looking for candidates from Hartford, Windsor Locks, Manchester, Norwich, Middletown, Windham, Waterbury, Torrington, and Danbury, CT.

Work Schedule:
Monday to Friday, 9 AM to 5 PM. Flexibility to accommodate the needs of families is essential, and working outside of these hours may be necessary.


The Outreach Care Specialist is responsible for providing individual and family support to ensure that members are connected to essential community services, resources, and care coordination. This role also emphasizes the importance of clear communication among the care team and treating clinicians to support members and their families. The specialist will coordinate member-specific care plans within the care network.

Collaboration is key, as the specialist will work alongside ICC staff, families, consumers, community collaborative members, stakeholders, and providers to ensure that the appropriate services are accessible to designated members. The ultimate goal is to promote independence and enhance the quality of life by assessing individual needs and facilitating access to suitable community services and supports.


Key Responsibilities:
1. Coordinate follow-up care plan needs for members by scheduling appointments or enrolling them in programs.
2. Assess member compliance with medical treatment plans through telephone outreach or on-site visits.
3. Identify barriers to compliance and coordinate resolutions.
4. Recognize opportunities that affect quality goals and suggest process improvements.
5. Recommend modifications to treatment plans and determine the need for additional services in collaboration with case management and providers.
6. Facilitate identification and referral to local, state, or federally funded programs.
7. Coach members on strategies to mitigate health risks.
8. Prepare reports to document case progress and compliance updates.
9. Establish and maintain relationships with relevant agencies as per contract requirements.
10. Perform other duties as assigned.

Minimum Qualifications:

A high school diploma is required, along with a minimum of one year of relevant experience; or an equivalent combination of education and experience.


Preferred Skills and Experience:
- Familiarity with community resources in the area.
- Bilingual candidates (Spanish) are preferred.
- A BS/BA degree in a related field is preferred.
- Experience with electronic health records (EHR) systems.

For URAC accredited areas, the following professional competencies apply: Associates in this role are expected to possess strong oral, written, and interpersonal communication skills, along with problem-solving, facilitation, and analytical skills.
