Job Description

The Agency Admissions Liaison plays a pivotal role in ensuring a seamless transition for incoming clients into the Aware Recovery Care program. This position collaborates closely with Admissions and Agency leadership to guarantee a smooth onboarding process. Key responsibilities include reviewing eligibility and program expectations prior to client meetings with care teams and the commencement of care. A thorough understanding of Aware Recovery Care's treatment philosophy, admission process, and criteria is essential for success in this role. Additionally, a basic knowledge of substance abuse and dual diagnosis is required.

Key Responsibilities
  • Communicate and collaborate with central Admissions and Agency Leadership to ensure a smooth transition for clients into Aware Recovery Care's care.
  • Scans, uploads, organizes, saves, and stores confidential documentation within the Electronic Health Record (EHR).
  • Participate in collaborative treatment team planning and coordination options, including setting goals for treatment and designating potential care teams based on client personality, age, gender, and history.
  • Coordinate client meet and greets with the Intake Coordination department.
  • Explain the program to patients and ensure they comprehend informed consent.
  • Explain Admissions documentation necessary for entry into the Aware Recovery Care program to clients and obtain verbal authorization while in client presence via telehealth.
  • Trained in basic Aware Recovery Care admission and referral processes.
  • Proficient in operating CRM and EHR software.
  • Verify contacts, clients, providers, institutions, and their hierarchies, as well as referrals in and external referrals made on a daily basis through Salesforce, ensuring compatibility with information in the EHR.
  • Understand and communicate the importance of treatment compliance.
  • Aware of resources available for co-morbidities, dual diagnosis if needed, as well as other levels of care and assist agency staff in making referrals.
  • Explain patient rights and responsibilities.
  • Learns all program options available to patients.
Requirements
  • Bachelor's Degree preferred.
  • Employee is expected to perform all duties within their scheduled work week.
  • Complies with State/Federal requirements.
  • Ability to use electronic health records, phone systems, customer resource management software, and more general office computer systems.
  • Professional and proficient written and verbal communication.

  • Able to maintain a cooperative and positive attitude at all times and approaches patients, colleagues, and family members in a professional and welcoming manner.
  • At least a basic understanding of Substance and Alcohol Use Disorders.
  • Employee is expected to perform all duties within their scheduled work week.

  • Professional and proficient written and verbal communication.
