Job Summary:

The Medical Social Worker II plays a vital role in assisting patients and their families/caregivers to cope with the social and emotional challenges associated with illness. Under the general direction of the Social Work Manager/LCSW, this position delivers age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure, as well as state and federal regulations.

Key Responsibilities:
  1. Provides psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem-solving, stress reduction, and developing healthy coping strategies in individual/family/group settings.
  2. Offers counseling for disease acceptance and understanding.
  3. Develops and implements individual Plans of Treatment to assist patients and families in coping with and/or restoring social, emotional, financial, and environmental factors affected by illness.
  4. Completes psychosocial assessments.
  5. Partners with patients to identify needs and develop and implement individual treatment plans based on mutually agreed-upon treatment plans.
  6. Discusses options for care proactively, including Kaiser resources and external community/government resources, to assist patients and families in developing short- and long-term care plans as appropriate.
  7. Collaborates with other disciplines in assessing, planning, and providing services for patients utilizing biopsychosocial information.
  8. Assists patients in advocating for themselves to receive appropriate services within Kaiser and in the community.
  9. Assists patients and families with care planning and discharge plans.
  10. Takes, reviews, evaluates, and prioritizes written and oral referrals.
  11. Maintains documentation, records, and data collections.
  12. Responsible for completing required documents in a complete and timely manner.
  13. Functions as part of the Skilled Nursing Facility Team to ensure appropriate, timely placement of Kaiser members in nursing facilities.
  14. Liaisons between patients and Kaiser, maintaining a positive relationship with Kaiser and providing for continuity of care.
  15. Identifies appropriate levels of care and facilities for referred patients, where applicable.
  16. Obtains placements, where applicable.
  17. Collaborates with internal and external resources in Kaiser and the community to meet mutually agreed-upon goals and objectives.
  18. Provides information and referral to community resources as requested.
  19. Coordinates exchange of information between Kaiser, families, members, and skilled nursing facilities.
  20. Determines application of Kaiser, Medicare, and Medi-Cal benefits to specific patient situations.
  21. Participates in Utilization Management/Quality Assurance activities.
  22. Assists in coordinating communication between regional offices, clinics, hospitals, and field staff, triaging of phone calls from members/families.
  23. Works with referral sources to clarify and complete required clinical and psychosocial information.
  24. Performs other related duties as necessary.
Requirements:
  • Experience: Step I - 0-2 years social work experience, Step II - 2-4 years social work experience within the last five years, Step III - 4 or more years social work experience within the last ten years.
  • Education: Masters in Social Work accredited by the Council of Social Work Education.
  • License, Certification, Registration: N/A.
Preferred Qualifications:
  • At least one year post-MSW experience in a healthcare setting preferred - MSW internship may be considered in lieu of this requirement.
  • LCSW preferred.
