Hospice Care Team Member
At HexaQuEST Health, Inc., we provide compassionate care to patients and their families during the final stages of their illness. Our Hospice teams support patients in their homes, skilled nursing facilities, assisted living facilities, and adult foster homes.
We offer a range of services, including intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained volunteers, and bereavement support.
Medications and equipment are included as needed.Requirements
Current Oregon RN licensure is required. Healthcare Provider Level AHA BLS certification is also necessary. A BSN and/or previous experience in hospice or case management are preferred.
Work Environment
This position requires the use of a personal automobile. A valid driver's license and auto liability/property damage insurance are required. The employee must maintain their vehicle in good working order.
Additional Information
This is a normal urgency position with days and every other weekend shifts. The employee must have a minimum of 1 year of experience and be able to work 40 hours per week. The specialty type is nursing, with a sub-specialty in hospice RN. BLS/BCLS certification is required, and bilingual skills are not necessary. The EMR used is EPIC, and holiday coverage is required.
