As a Hospice Home Care Team Member, you will play a vital role in delivering comprehensive care to patients and their families in their homes, making a meaningful impact on their lives.

Responsibilities
  • Develop and coordinate patient care plans in collaboration with an interdisciplinary team.
  • Ensure effective communication with patients, families, physicians, and colleagues through verbal and written interactions.
Requirements
  • Current licensure as a Registered Nurse in New York State.
  • Associate Degree in Nursing or BSN, with at least 1 year of experience as an RN, preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and reliable vehicle.
Compensation and Benefits

We offer a competitive salary of $65,000 - $85,000 per annum, depending on experience, as well as:

  • National pre-taxed mileage reimbursement.
  • Meetings and charting conducted via supplied laptop.
  • Stipend for cell phone provided.
  • Comprehensive orientation program.
  • Full benefits package through St. Peters Health Partners.
