Job Overview

The Director of Social Services at Windsor Health & Rehabilitation Center LLC plays a crucial role in overseeing the comprehensive management of the psychosocial needs of residents. This position requires the ability to provide counseling or collaborate with psychiatric professionals to enhance patient well-being and progress.

Key Responsibilities

The individual in this role is entrusted with the following responsibilities:

  • Reviewing policies and procedures to ensure social services are delivered in accordance with federal, state, and local regulations.
  • Conducting interviews, assessments, and developing tailored plans to address the unique needs of residents.
  • Evaluating the facility's social service program and providing recommendations to the administration.
Essential Duties

Key duties include, but are not limited to:

  1. Completing admission assessments for new residents.
  2. Conducting interviews with incoming patients.
  3. Fulfilling MDS sections as required.
  4. Completing ASCEND documentation.
  5. Developing care plans as necessary.
  6. Providing ongoing support to residents, including conducting 72-hour meetings and documenting care plans.
  7. Investigating allegations of mistreatment and grievances appropriately.
  8. Planning for discharges, ensuring living arrangements are confirmed prior to patient release.
  9. Addressing financial concerns through discussions with patients and their families.
  10. Intervening in family crises on behalf of patients.
  11. Offering counseling for significant events impacting residents.
  12. Submitting ASCEND documentation to relevant agencies upon admission.
  13. Assisting residents in obtaining level of care extensions.
  14. Collaborating with the Business Office Manager to facilitate benefit approvals for long-term care residents.
Qualifications

To be considered for this position, candidates should possess:

  • A Master’s degree in Social Work (MSW).
  • A minimum of 2 years of experience in a Long-Term Care environment.
  • In lieu of a Master’s degree, a Bachelor’s degree in Social Work with 5 years of relevant experience.
  • Flexibility in scheduling.
  • Strong communication skills with the ability to resolve complaints and address concerns effectively.
