Job Description

Mental Health Cooperative, Inc. is a leading provider of mental health services in Middle and East Tennessee. We are seeking a highly skilled Therapist to join our team.

Job Summary

The Therapist will work with individuals and families in the home setting to promote optimum mental health. This includes helping individuals deal with addictions and substance abuse, family and parenting problems, suicide, stress management, problems with self-esteem, and issues associated with mental and emotional health.

Key Responsibilities
  • Work with individuals and families in the home setting to promote optimum mental health
  • Help individuals deal with addictions and substance abuse, family and parenting problems, suicide, stress management, problems with self-esteem, and issues associated with mental and emotional health
  • Implement System of Care and Wraparound principles
  • Encourage consumers to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships
  • Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies
  • Refer consumers and family members to community resources or to specialists as necessary
  • Meet with families, clinic providers, care managers and other team members in order to exchange necessary information during the treatment process
  • Collaborate with other staff members and agencies to perform clinical assessments and develop treatment plans
  • Participate in SOCAT team on call rotation
  • Participate in team-based supervision meetings
  • Collaborate with program supervisor to train staff; provide field case consultation and support to staff
  • Participate and engage in the local SOCAT team
  • Responsible for attending all mandatory in-services, continuing education and annual health requirements
  • Comply with all Corporate Compliance policies, practices and procedures
  • Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director
  • Comply with all OSHA and safety policies, practices, and procedures
  • Will be compliant with the relevant regulations concerning the privacy and security of consumer's protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
  • Follow all policies and procedures set by the Agency
Requirements
  • Demonstrates effective communication skills with referral sources, consumers and other collateral contacts involved in consumer care as evidenced by courteous demeanor and positive attitude with consumers and collateral contacts
  • Documentation is completed in a timely manner, thorough, follows format, satisfies requirements, and outlines plan of care and appropriate notification of treatment team of proposed plan of care
  • Assessments include appropriate integration of historical data and clinical data to formulate a clear and accurate diagnosis
  • Displays sensitivity and responsiveness to consumer's culture, age, gender and disabilities
  • Demonstrates ability to provide high quality evidenced based interventions and they are implemented appropriately as evidenced by applying theoretical basis of intervention and general strategies related to client care
