Job SummaryThe successful candidate will provide high-quality patient care in the comfort of their own homes, working closely with a team of healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Conduct thorough assessments of patients' medical needs and develop personalized care plans
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination
- Monitor patients' progress, making adjustments to care plans as needed
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans
- Graduate of an accredited nursing program
- Active, unrestricted nursing license in the state of South Carolina or compact multi-state license
- Minimum of one year of nursing experience, preferably in home healthcare
- Med/Surg or Critical Care/IV Therapy experience preferred
- Clinical proficiency in nursing specialty, strong communication skills, and compassion
