Role Responsibilities:
We are in search of a full-time Substance Use Support Specialist to deliver essential services to pregnant and parenting individuals with a history of substance use through culturally sensitive, community-focused, multi-generational case management and support group facilitation.

In this role, you will:
  • Conduct thorough client evaluations to pinpoint strengths and challenges affecting family stability, including employment, housing, healthcare, and emotional/social hurdles that may impact child safety and well-being.
  • Develop personalized case plans aimed at providing guidance and support, focusing on goals related to sobriety, prenatal and postnatal care, parenting skills, and child development. Facilitate access to necessary resources in line with the case plan to bolster family self-sufficiency.
  • Coordinate services with Child Protection, Public Health, Community Corrections, Mental Health, and Chemical Dependency treatment providers, as well as other relevant community organizations.
  • Engage in outreach initiatives to cultivate and sustain a network of resources while advocating for families.
  • Co-facilitate weekly or bi-weekly sober support and educational groups, in addition to organizing parent-child activities as scheduled.
  • Complete all required documentation promptly, which may include time reporting, case notes, and family support plans.
Candidate Profile:
The ideal candidate will possess strong critical thinking abilities and excel in a collaborative team environment. A bachelor's degree in a human services-related field is mandatory. Preference will be given to candidates with experience in substance use, child welfare, direct engagement with diverse populations, and bilingual capabilities. Additionally, preference will be extended to those holding a Minnesota Social Work License or license eligibility.
