Under the general administrative guidance of the Director of Business Development, the Community Outreach Specialist will promote hospice and home health services to healthcare professionals, the community, and individuals or families in a manner that exemplifies the quality of the organization.

This role involves building and nurturing professional relationships with all referral sources, including hospitals, physician practices, nursing homes, hospital case managers, discharge planners, assisted living facilities, and other relevant entities.



  • Develops and sustains professional connections with all referral sources, such as physicians, nursing homes, assisted living facilities, and hospital case managers.
  • Ensures adherence to all state, federal, and local regulatory standards.
  • Educates the community about home care services and Grane Home Health & Hospice.
  • Engages daily with professionals in the healthcare sector, including physicians, discharge planners, medical social workers, case managers, and skilled nurses.
  • Advocates for Grane Home Health & Hospice's programs and services to medical professionals and community organizations.
  • Bachelor's degree in Business, Healthcare, or a related field is preferred.
