Mental Health Counselor

To provide intensive in-home services to children and adolescents at risk for hospitalization, preventing hospitalization and maximizing community functioning.
Job Summary:
The IICAPS Mental Health Counselor provides services to families in their homes and out in the community, working a flexible schedule to accommodate family needs.
Key Responsibilities:
Collaborate with clinicians in assessment, treatment planning, and stabilization of clients.
Offer therapeutic support and parent guidance to families.
Carry a caseload of 8-9 cases, averaging 5.5 hours of direct service per week per case.
Work individually with children as clinically indicated.
Collaborate with clinicians to provide advocacy, case management, and concrete services in accordance with treatment plans.
Additional Duties:
Participate in supervision and case presentations.
Provide crisis resolution and collaborate with other service providers for continuity of care.
Write relevant reports, progress notes, and other documentation as needed.
Qualifications:
BS in social work or related mental health field or equivalent experience.
Prior experience in Human Services, Health Care/Mental Health, Education, Community Organizing, or Advocacy.
Experience working with children and adults, and comfort working with families in their homes.
Valid driver's license, good driving record, and reliable transportation.
Preferred Qualifications:
Bi-lingual (Spanish) desired.
Working Conditions:
Evening and occasional weekend hours required, with flexibility to accommodate family schedules.
