Job Summary

As a Clinical Social Worker at Yale New Haven Health, you will be part of a team dedicated to providing quality medical care and treatment that is coordinated and centered on the patient's specific needs. You will work collaboratively with members of the healthcare teams throughout the organization to provide comprehensive care and support to patients and their families.

Responsibilities

• Conduct diagnostic assessments, counseling, and psychotherapy to address patients' mental health needs
• Collaborate with interdisciplinary teams to develop and implement treatment plans
• Provide education and support to patients and their families on mental health topics and coping strategies
• Maintain accurate and confidential records of patient interactions and treatment plans

Qualifications

• Master's degree in social work from an accredited program
• 3,000 hours of post-masters social work experience and 100 hours of post-masters clinical supervision (for Connecticut LCSW licensure)
• Licensed Clinical Social Worker (LCSW) from the State of Connecticut or Licensed Independent Clinical Social Worker (LICSW) from the State of Rhode Island
• Strong organizational and communication skills, with the ability to work effectively in a fast-paced environment
