Travel Infusion Nurse Practitioner

This is a per diem travel position for a Nurse Practitioner with AleraCare, a provider of ambulatory infusion treatments and specialty pharmacy services. The role involves travel throughout the Los Angeles metro area and the San Francisco Bay Area.
Job SummaryWe are seeking a highly skilled and experienced Nurse Practitioner to join our team as a Travel Infusion Nurse Practitioner. The successful candidate will be responsible for providing nursing services to assigned patients in accordance with their plan of care.
Responsibilities- Maintain patient confidentiality at all times.
- Function as a patient advocate, treating patients and families with respect and dignity.
- Perform initial patient evaluations/assessments and develop a written plan of care.
- Prepare clinical and progress notes, coordinate services, and inform the physician and other personnel of changes in the patient's condition and needs.
- Responsible for the clinical record of each patient receiving nursing care.
- Ensure documentation meets all standards and policies, including documentation of all findings, interventions, and results.
- Collaborate with physicians and other members of the healthcare team to provide optimal care for the patient.
- Evaluate and reevaluate the patient and/or family for changing needs for services.
- Provide patient and/or family with health education, counseling, and referrals to other healthcare professionals and/or community resources when appropriate.
- Order and/or interpret lab work and diagnostic tests according to patient need and/or plan.
- Demonstrate the ability to prescribe medications and non-pharmacological therapies based on the patient's diagnoses, health history, and age.
- Initiate case conferences and communicate the patient plan of care with team members.
- Participate in the company's Performance Improvement and Continuous Quality Improvement (CQI) programs.
- Participate in committees when requested.
- Attend continuing education programs and staff meetings as appropriate to job responsibilities.
- Assume responsibility/ownership of clinic location(s).
- Support and maintain a culture of safety and quality care.
- Stay current with medical advancements, new technology, new drugs through inservices, continuing education classes, society meetings, professional journals, etc.
- Masters Degree from an accredited Nurse Practitioner Program.
- Holds a current NP state license in the state of practice.
- At least 4-5 years of advanced practice registered nursing (APRN) experience, ambulatory and infusion nursing experience is preferred.
- Strong interpersonal, time management, and organizational skills.
- Strong written and verbal communication skills.
- Experience with electronic medical records (EMR).
- Ability to recognize and direct information to the appropriate healthcare provider.
- Computer skills that include word processing and efficient use of the internet and e-mail.
- Current CPR card, advanced practice certification, current APRN licensure, valid drivers license, reliable transportation, automobile insurance.
