Job Summary:

We are seeking a skilled Community Mental Health Nurse Specialist to join our team at Trinity Health. As a key member of our community-based care approach, you will provide compassionate care to individuals with mental health needs.

About the Role:

This part-time position involves delivering person-centered care in a variety of settings, including community homes, clinics, and hospitals. You will work collaboratively with our treatment teams to develop and implement individualized treatment plans that address each client's unique needs and goals.

Responsibilities:

  • Provide direct care services, including medication education, case management, counseling, and advocacy.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients.
  • Develop and implement treatment plans based on client assessment and need.
  • Link clients to essential resources and services, such as housing, employment, and educational opportunities.
Requirements:

To be successful in this role, you will need:

  • A valid Registered Nurse license and at least 1 year of nursing experience (BSN preferred).
  • Valid driver's license and ability to drive an organization vehicle.
  • Act 33/34/73 Clearances (Criminal/Child/FBI Department of Human Services) and Child Protective Services Law training (CPSL).
  • Pre-employment Physical/TB/Hep B and Pre-Employment Drug Screen.
