We are seeking a highly skilled and dedicated PACT Resource Specialist to join our multidisciplinary team. As a key member of our team, you will play a vital role in providing rehabilitation and support services to individuals with serious mental illness.

Key Responsibilities
  • Participate in comprehensive assessments and treatment planning for individuals with serious mental illness.
  • Provide rehabilitation and support services to individuals, including education, employment, and social development.
  • Collaborate with community agencies and families to maintain coordination in the treatment process.
  • Participate in daily organizational meetings and individual treatment team meetings.
  • Provide on-call crisis intervention and support services.
  • Perform other duties as required.
