Re-Entry Case Worker

Job Summary

The Re-Entry Case Worker will be part of a multi-disciplinary team at the Old Colony YMCA responsible for conducting in-reach to Massachusetts Correctional Institutions and County Houses of Corrections to publicize and enroll clients in services available through the Ralph Gants Reentry Services Program.

Services include education, career counseling, treatment, housing support, and community-based referrals. The Re-Entry Case Worker will build rapport, demonstrate empathy, set professional boundaries, and deliver support to ensure those returning from incarceration find positive change.

This role requires strong oral, written, and presentation skills, as well as computer proficiency in MS Office. The ideal candidate will have a Bachelor's degree or equivalent experience and a minimum of two years' experience working with justice-involved individuals.

Key Responsibilities
  • Conduct in-reach to Massachusetts Correctional Institutions and County Houses of Corrections to publicize services available through the Ralph Gants Reentry Services Program.
  • Develop rapport with individual people returning to the community after incarceration to facilitate a 'warm-handoff' and provide a smooth reentry experience.
  • Manage the caseload of all individuals participating in the RGRSP at the Old Colony YMCA.
  • Work with the multi-disciplinary team to engage clients in appropriate services and coordinate with community-based resources to address needs.
  • Maintain contact with clients for 18 months post-release.
  • Provide information about mentoring training and facilitate aftercare or alumni groups for CJSC clients.
Requirements

The ideal candidate will have a strong understanding of community resources, including housing, healthcare, and food services. They must be willing to work with high-risk populations and have excellent organizational and case management skills.
