We are looking for a Family Support Case Manager to deliver essential stabilization services to families transitioning from shelter environments or those referred by our Front Door program. This role involves providing comprehensive case management support for a duration of three years, focusing on ongoing barrier reduction, effective communication with landlords, and facilitating appropriate referrals to ensure families maintain stable housing post-stabilization.
The Family Support Case Manager will employ a trauma-informed methodology, which includes crafting monthly Rehousing Plans, connecting families with community resources, advocating for their needs, conducting assessments, and providing crisis intervention. The ultimate goal is to assist families in securing permanent housing solutions.

Key Responsibilities:
- Conduct intake and needs assessments for each family, covering various aspects such as housing, financial management, employment, education, nutrition, legal matters, health, parenting, and daily living skills.
- Interview clients to develop tailored Individualized Service Plans that align with program objectives.
- Assist families in finding permanent housing by leveraging all available resources, including private rental markets and long-term subsidy programs.
- Collaborate closely with families to identify strengths and create strategies to overcome barriers identified during assessments.
- Ensure compliance with EOHLC Shelter Scope of Services.
- Support clients in building self-sufficiency plans.
- Facilitate clients' transition to self-advocacy roles.
- Conduct home visits and provide transportation for clients as needed.
- Manage emergency response situations effectively.
- Set and monitor client goals.
- Work collaboratively with an interdisciplinary team of service providers and maintain accurate case records.
Qualifications:
- High School Diploma or Equivalent required.
- Prior relevant experience is essential.
- A minimum of one year of experience in assessment, advocacy, and case management within the Human Services sector is required.
- Experience working with low-income, culturally diverse, and at-risk families and individuals is preferred.
- Strong knowledge of community resources available to low-income families.
- Exceptional customer service, organizational, and communication skills, both oral and written.
- Ability to manage multiple priorities in a fast-paced environment.
- Proficient in basic computer software and data collection systems.
- Strong crisis management abilities.
- Capacity to maintain accurate records and uphold confidentiality.
- Meticulous attention to detail.
- Valid Driver's License is required.
- Bilingual skills (English/Spanish, English/Haitian-Creole, English/Khmer, English/Portuguese) are preferred.
