We are seeking a compassionate and skilled Licensed Practical Nurse/Licensed Psychiatric Technician/Licensed Vocational Nurse to join our team at All Ways Caring HomeCare. As a key member of our care team, you will provide high-quality patient care and support to our clients in their homes.
Key Responsibilities- Provide Personal Care: Assist clients with daily living activities, such as bathing, dressing, and grooming, while maintaining their dignity and independence.
- Monitor Vital Signs: Regularly check clients' vital statistics, including temperature, blood pressure, and oxygen levels, and report any abnormalities to the Case Manager and Physician.
- Administer Medications: Safely administer medications, feedings, and other treatments as needed, and document the results of these interventions.
- Document Care: Accurately record all nursing actions, communication with family and healthcare professionals, and progress towards established goals.
- Maintain a Safe Environment: Ensure infection control policies are followed, report any potential communicable diseases or infections, and maintain the overall cleanliness and organization of the client's home.
- Coordinate Care: Order supplies, medications, and feedings as needed, and ensure that all patient needs are met in accordance with the instructions of the physician.
- Respond to Incidents: Monitor incidents involving clients, complete incident reports, and inform Case Managers and healthcare professionals of any changes in the client's condition.
- Licensure: Current licensure as a Licensed Practical Nurse/Licensed Psychiatric Technician/Licensed Vocational Nurse in the state of employment.
- Experience: Previous experience in a healthcare setting, preferably in a home care or hospice environment.
- Skills: Excellent communication and interpersonal skills, ability to work independently, and strong attention to detail.
