Infusion Nurse Practitioner

AleraCare is seeking a skilled Infusion Nurse Practitioner to join our team at our San Jose, CA infusion center. As a key member of our healthcare team, you will provide high-quality patient care and contribute to the success of our organization.
Job SummaryThe Infusion Nurse Practitioner will be responsible for providing nursing services to assigned patients in accordance with their plan of care. This includes conducting initial patient evaluations, developing and implementing care plans, and collaborating with healthcare teams to ensure optimal patient outcomes.
Responsibilities- Provide patient-centered care, maintaining confidentiality and treating patients and families with respect and dignity.
- Conduct initial patient evaluations, assess patient needs, and develop written care plans.
- Collaborate with healthcare teams to ensure seamless care coordination and optimal patient outcomes.
- Document patient progress, maintain accurate records, and communicate effectively with patients, families, and healthcare teams.
- Participate in Performance Improvement and Continuous Quality Improvement (CQI) programs to enhance patient care and safety.
- Stay current with medical advancements, new technologies, and best practices through ongoing education and professional development.
- Master's Degree from an accredited Nurse Practitioner Program.
- Current NP state license in the state of practice.
- At least 4-5 years of advanced practice registered nursing (APRN) experience, with ambulatory and infusion nursing experience preferred.
- Strong interpersonal, time management, and organizational skills.
- Excellent written and verbal communication skills.
- Experience with electronic medical records (EMR) and ability to maintain accurate patient care records.
