Infusion Nurse Practitioner

AleraCare, a leading provider of infusion treatments and specialty pharmacy services, is seeking a highly skilled Infusion Nurse Practitioner to join our team in San Jose and Mountain View. Our clinics are open weekdays, Monday through Friday, from 8:00am to 5:00pm. We offer a competitive compensation package and a comprehensive benefits program to our full-time providers.
ResponsibilitiesThe successful candidate will be responsible for providing high-quality nursing services to assigned patients, in accordance with their individualized plan of care. Key responsibilities include:
- Maintaining patient confidentiality and upholding the highest standards of professionalism.
- Functioning as a patient advocate, treating patients and families with respect and dignity.
- Providing care in accordance with professional standards and state regulations.
- Conducting initial patient evaluations and assessments, developing written plans of care, and documenting patient progress.
- Collaborating with physicians and other healthcare professionals to ensure optimal patient care.
- Participating in Performance Improvement and Continuous Quality Improvement (CQI) programs.
- Staying current with medical advancements, new technologies, and best practices through ongoing education and training.
To be considered for this exciting opportunity, candidates must possess:
- A Master's Degree from an accredited Nurse Practitioner Program.
- A current NP state license in the state of practice.
- At least 4-5 years of advanced practice registered nursing (APRN) experience, with a preference for ambulatory and infusion nursing experience.
- Strong interpersonal, time management, and organizational skills.
- Excellent written and verbal communication skills.
- Experience with electronic medical records (EMR) and computer skills, including word processing and internet navigation.
The physical demands of this role include:
- Regularly standing, walking, sitting, talking, and hearing.
- Occasional lifting and moving up to 50 pounds.
- Specific vision abilities required include close vision, distance vision, peripheral vision, and the ability to adjust focus.
