About the Role

We are seeking a compassionate and skilled Hospice Supportive Care Nurse to join our team at Hospice Of Hope, Inc. As a member of our care team, you will provide one-to-one care to terminally ill patients in their private residences, nursing homes, and assisted living facilities in Kentucky and Ohio.

Key Responsibilities:

  • Provide end-of-life care in accordance with individualized plans of care, demonstrating the ability to assess and respond to patient and family needs in a timely manner.
  • Educate patients and caregivers on end-of-life services and resources, promoting a supportive and informed environment.
  • Document complex medical information using clinical tools and data to complete assessments of patient needs.
  • Establish professional relationships with patients, families, and providers, utilizing strong written and verbal communication skills.
  • Basic computer skills, including working knowledge of the Microsoft Office suite, are required.
  • Work independently, with minimal direct supervision, to provide care in various settings.
Requirements

Professional Qualifications:

  • Graduate of an accredited School of Nursing and current licensure, or eligibility to be licensed, in Kentucky and Ohio.
  • At least one year of experience in a patient care position in a hospital, nursing home, or related healthcare facility is preferred.

Personal Qualities:

  • Empathy, responsibility, and dependability are essential personal qualities for this role.
  • Effective communication skills are necessary for working with team members, patients, providers, and family members.

Physical Job Requirements:

  • Must be able to arrange travel and access client homes, nursing homes, hospitals, and other facilities independently.
  • Must demonstrate proper use of personal protective equipment and observe universal precautions.
  • Must be able to lift at least 50 pounds using proper body mechanics.
  • Must have a valid driver's license, current auto insurance, and reliable transportation.
