The Haven Texas is a leading provider of detox and stabilization services for individuals experiencing substance abuse withdrawals. Our 5-7 day programs are located within acute care hospitals, offering comprehensive treatment options.

We accept Medicare, Medicaid, and are in-network with all major insurance providers, ensuring seamless coverage for our clients. Additionally, we provide 24-hour admission and transportation services for those in need.

Job Summary

We seek an experienced Discharge Coordinator / Discharge Planner to join our team. This role involves continuous assessment of patient status and thorough documentation of pertinent information. You will also be responsible for accurate documentation of communication and coordination for the patient's discharge and transition to the next level of care.

As a key member of our team, you will serve as an information resource to patients and maintain consistent and effective communication with all departments within the unit. Your expertise will help match resources and referrals to meet the unique needs of our patients.

Requirements and Qualifications

  • Licensed Chemical Dependency Counselor (LCDC); or
  • Licensed Professional Counselor Associate (LPC-Associate); or
  • Licensed Master Social Worker (LMSW)

A valid Texas Driver's License is also required. Preference will be given to candidates with experience in discharge planning or supporting patient transitional care.

Additionally, individuals in recovery with at least one year of sobriety are strongly encouraged to apply.

Compensation and Benefits

The estimated annual salary for this position is $55,000-$65,000, depending on experience. We offer a competitive benefits package, including health insurance, retirement savings plan, and paid time off.
