Position Overview

Aveanna Healthcare is a leading provider of home health care services, and we are currently seeking a skilled Physical Therapist to join our dynamic team.

In this role, you will have the opportunity to make a meaningful difference in the lives of our patients by providing high-quality physical therapy services in the comfort of their own homes.

Key Responsibilities:
  • Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
  • Develop and implement individualized plans of care to achieve patient goals and outcomes.
  • Provide physical therapy services to comply with prescribed physician orders.
  • Maintain accurate and timely documentation of patient progress and outcomes.
  • Collaborate with healthcare team members to ensure seamless care coordination.
Requirements:
  • Master's degree in Physical Therapy from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapist in the state of practice.
  • Valid CPR and driver's license.
Preferred Qualifications:
  • Strong organization and communication skills.
  • Attention to detail and time management skills.
  • Proven relationship-builder with experience in home health therapy and OASIS documentation.
