Hospice Admissions Specialist

We are seeking a highly skilled Care Coordinator for Hospice Services to join our team in Middlebury, CT. As a Care Coordinator, you will play a vital role in supporting patients and their families through the hospice care process, ensuring that they receive the highest level of care and compassion.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate with healthcare providers and other stakeholders to ensure seamless care delivery
- Provide emotional support and counseling to patients and their families
- Maintain accurate and detailed records of patient interactions and care plans
Requirements:
- 1+ year of experience in hospice care or a related field
- Excellent communication and interpersonal skills
- Demonstrated ability to work effectively in a fast-paced environment
