Job Description

This Admissions Specialist role is responsible for coordinating admissions for patients into our hospice care program. The successful candidate will have excellent communication skills and be able to build strong relationships with patients, families, and healthcare providers.

Responsibilities

  • Assess patient needs and develop individualized care plans
  • Coordinate admissions and transfers between facilities
  • Communicate with patients, families, and healthcare providers to ensure seamless transitions
  • Maintain accurate records and documentation

Requirements

  • 1+ year experience in hospice care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong organizational and time management skills

Salary Range

The estimated salary for this role is $40,000 - $55,000 per year, depending on experience.
