Palliative Clinical Care Specialist

The University of Vermont Health Network is seeking a highly skilled Palliative Clinical Care Specialist to join their team. This role involves providing expert social support and care coordination services to patients with serious illnesses and their families.
The ideal candidate will possess a Master's degree in Social Work (MSW) from an accredited school, along with at least three years of experience in clinical social work. Preference will be given to candidates with advanced certification in palliative care or hospice.
This is an on-site position located at Porter Medical Center in Middlebury, VT. The salary for this role is approximately $52,500 - $80,750 per year, based on experience.
About the RoleThis Palliative Clinical Care Specialist will function as part of a multi-disciplinary team, providing ongoing continuity of care and engaging psychological, social, and bereavement support for patients and their families.
The successful candidate will have excellent communication skills, both written and verbal, and demonstrate a strong ability to build honest and trustworthy relationships with individuals from diverse backgrounds.
