Case Manager

Indiana Health Centers, Inc. is seeking a dedicated Case Manager to join our team at our Michigan Street location. As a Case Manager, you will play a vital role in assisting clients in accessing community and healthcare resources, while ensuring continuity of care.
The ideal candidate will possess excellent interpersonal and customer service skills, with the ability to work effectively in a fast-paced environment. A strong knowledge of available community resources and a passion for social services are essential for this role.
- Assess client needs and develop personalized plans to ensure access to medical care and local resources
- Collaborate with clinic staff and community agencies to ensure seamless care coordination
- Provide orientation and in-service training to community agencies
- Maintain accurate participant records to aid service planning and delivery
- Prepare and submit reports as needed
- Hourly position (based on education and experience)
- $1000.00 retention bonus paid after one year of employment

- Education: High school diploma or equivalent required (Bachelor's degree preferred)
- Certification: May require to complete Indiana Navigator certification based on clinic needs
- Experience: Three years of experience in case management in an FQHC or healthcare setting
- Driver's License: Must possess an Active and valid Indiana Driver's License, including State-mandated Auto Insurance
- Travel: The ability to travel throughout the community is required
- Knowledgeable of available community resources
- Excellent interpersonal and customer service skills
- Bilingual preferred, not required
- Computer-proficient
