CVS Health is seeking a dedicated and compassionate RN Field Case Manager to join our team in Southwest Michigan. As a key member of our care team, you will play a vital role in elevating patient care to a whole new level.

Job Summary

We are looking for a skilled and experienced RN to work at home with 25-50% travel within Southwest Michigan. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.

Responsibilities
  • Conduct comprehensive assessments of members' medical needs and develop proactive plans of care to address identified issues.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to address complex health and social determinants.
  • Utilize clinical judgment and motivational interviewing skills to ensure maximum member engagement and discern their health status and health needs.
  • Develop and implement case management processes in compliance with regulatory and company policies and procedures.
Requirements
  • Minimum 3+ years of clinical experience as an RN.
  • Active and unrestricted Michigan RN licensure.
  • Ability to travel up to 25-50% of the time within Southwest Michigan.
Preferred Qualifications
  • 2+ years of experience in case management, discharge planning, and/or home health care coordination.
  • Behavioral health experience.
  • Bilingual in Spanish or Arabic.
  • Certified Case Manager.
  • Additional national professional certification (CRC, CDMS, CRRN, COHN).
