Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Angels of Care Pediatric Home Health. As a LPN, you will provide direct, one-on-one medical care to infants, children, and adolescents in their home, working closely with our interdisciplinary team to ensure the best possible outcomes for our patients and their families.

Responsibilities
  • Focus on the medical needs and treatment of patients, following the active plan of care provided by the physician and other healthcare professionals.
  • Conduct ongoing patient assessments and administer prescribed medication, treatments, and therapies as needed.
  • Coordinate care with other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
  • Educate family members on patient clinical care to enhance positive outcomes and promote preventative initiatives to protect quality of care.
