Job Summary

We are looking for a dedicated and compassionate Licensed Clinical Social Worker to provide exceptional behavioral healthcare services to clients of all ages. As a key member of our team, you will work collaboratively with primary health care providers, specialists, psychiatrists, dependency programs, and other healthcare professionals to deliver outstanding care.

Responsibilities
  • Provide accurate and timely communication to ensure correct care is provided.
  • Serve as clinician-of-the-day by being available for on-call during scheduled workdays for patient emergencies.
  • Assess patients and develop a list of actual and potential needs/diagnoses using advanced specialty knowledge, clinical skills, and expertise.
  • Gather health history of patients through interviews with patients and/or family members.
  • Plan and implement patient care using advanced knowledge, skills, and expertise.
  • Maintain patient documentation in a prompt and comprehensive manner.
  • Return patient calls and handle patient follow-ups for referrals promptly.
Requirements
  • Twelve (12) continuous months of experience within the last three (3) years rendering direct patient care in public or private institutions.
  • Valid Photo ID, Driver's License, or Passport.
  • Possess current Basic Life Support (BLS) certification from an American Heart Association.
  • Valid California Social Worker license.
