About the Nursing Role

HopeHealth is in search of a skilled Registered Nurse to join our dedicated team. This position involves providing hospice care to patients in their residences across southern Rhode Island. As a rapidly growing and innovative non-profit healthcare organization, HopeHealth is committed to delivering compassionate care for individuals facing serious health challenges. We offer a comprehensive range of medical services, care management, and support throughout the region.

This is a part-time role, requiring 24 hours per week, with a schedule from 8:00 AM to 4:30 PM and a rotating weekend obligation of up to every other weekend.

Eligible candidates may receive a bonus of up to $2,500, along with a benefits package.

Key Responsibilities:
  • Perform assessments for patients in home and nursing home settings, evaluating both patient and family needs from physiological, psychosocial, spiritual, and emotional perspectives.
  • Collaborate with the interdisciplinary team to create a comprehensive written care plan that outlines goals and nursing interventions based on assessment findings.
  • Execute the care plan under the supervision of the attending physician.
  • Regularly review and adjust the care plan to address the evolving needs of hospice patients and their caregivers.
  • Educate caregivers on the management of terminally ill patients, tailored to their individual backgrounds and experiences.
  • Exhibit strong interpersonal skills in interactions with hospice patients, families, nursing home personnel, colleagues, and community support services.
  • Maintain precise documentation of observations, issues, and nursing actions reflecting the care provided to hospice patients and families during visits.
Qualifications:
  • Preferred two years of experience in hospice, oncology, or homecare nursing, along with two years in medical/surgical nursing.
  • Strong nursing assessment and clinical skills are essential.
  • Current RN licensure is required; BSN is preferred.
  • Certification in hospice or palliative care nursing is a plus.
  • Must hold a valid nursing license in Rhode Island.
  • CPR certification is required.
