Position Overview

Communitas, Inc. is seeking a dedicated RN Health Care Coordinator to enhance the support services provided to individuals residing in our community group homes.

Key Responsibilities

The Health Care Coordinator will oversee the comprehensive healthcare and wellness of individuals in our Residential Program group homes and independent living settings. We are looking for a candidate with substantial experience working with adults with developmental and intellectual disabilities. Responsibilities include:

  • Training staff on specific treatment protocols.
  • Consulting on the medical needs of residents.
  • Exhibiting effective communication and sound judgment skills.
  • Collaborating as a team player with a strong work ethic.

While certification as a MAP Trainer is advantageous, training will be provided for the right candidate.

  • The annual salary for this role is $91,520.
  • This is a full-time position requiring 40 hours of work per week.
  • Standard working hours are Monday through Friday, with potential evening or weekend shifts for emergency healthcare needs.
  • Flexibility in scheduling is essential to meet the agency's and individuals' requirements.
  • Support for staff via text or phone is a necessary aspect of the role.

Qualifications and Skills

  • Current Massachusetts Registered Nurse (RN) license is required.
  • A minimum of 3 to 5 years of experience in a community health environment, preferably with adults with developmental and intellectual disabilities.
  • Respect for the dignity of individuals with disabilities and a commitment to their self-determination and growth.
  • A valid driver's license, a clean driving record, and reliable transportation are mandatory.
  • Successful candidates must complete a background check, including references, CORI, and fingerprinting.

Compensation Details: $44-44 Hourly Wage
