About This Career Opportunity
We are seeking a skilled Health Care Coordinator to join our team. As a Health Care Coordinator, you will be responsible for:
- Providing direct resident care and administering medications in accordance with the care plan.
- Training Medication Assistants and overseeing the Medication Program.
- Ensuring necessary documentation is completed to meet regulatory requirements.
- Assisting with admission and discharge processes.
- Performing assessments of residents and following up with the medical team as needed.
Requirements
To be successful in this role, you will need:
- A current California RN or LVN license.
- Minimum 2 years of experience in long-term care, home care, or a related field.
- Effective English verbal and written communication skills.
- Attention to detail.
- Working knowledge of computers and typing.
