Job Title: Behavioral Health Care Manager I, Child

Mental Health Cooperative is seeking a highly skilled Behavioral Health Care Manager I, Child to join our team. As a key member of our care management team, you will provide comprehensive care management services to child and adolescent consumers with severe emotional disorders (SED) and their families/supports.

Responsibilities:
  • Conduct ongoing assessments of consumers' strengths, needs, supports, and goals.
  • Meet with consumers and their supports in the community to provide support and guidance.
  • Collaborate with consumers and their supports to develop and implement comprehensive and individualized care plans, crisis plans, and other necessary tools.
  • Provide education and support to consumers and their supports regarding proper usage of emergency services, primary care physicians, and other community resources.
  • Ensure timely and efficient transitions of consumers within the agency and the community.
  • Meet productivity standards and participate in regular supervision and team meetings.
Requirements:
  • Bachelor's degree in a health-related field, such as counseling, psychology, or social work.
  • Valid Tennessee driver's license and acceptable motor vehicle report.
  • Acceptable criminal background investigation and personal automobile insurance.
  • Cell phone and transportation that seats 4 people.
