About the Role
We are seeking an experienced Veterans Care Specialist to join our team at the Department of Veterans Affairs. As a key member of our healthcare team, you will play a vital role in ensuring that our veterans receive the highest quality care.

Your responsibilities will include coordinating care for veterans with complex needs, working collaboratively with other healthcare professionals, and promoting best clinical outcomes and quality of life. You will also lead huddles and team meetings, perform various tasks such as EKGS and vision tests, and maintain databases and registries.

The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a team environment. They will also be responsible for promoting non-face-to-face care, calling patients post-discharge, identifying high-risk patients, and overseeing LVN practice as the clinic charge nurse on rotation. The estimated salary for this role is around $95,000-$175,000 per year.
