About the Role

We are seeking a highly skilled Pharmacist to join our team at PharMerica. As a key member of our healthcare team, you will be responsible for evaluating physician medication orders, making recommendations for therapy changes, and ensuring the safe procurement, storage, and use of medications.

Responsibilities
  • Evaluate physician medication orders for appropriateness, potential interactions, and route of administration
  • Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication
  • Collaborate with the healthcare team to provide clinical and technical support as needed
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals
  • Document all communication with physicians, nurses, and other members of the healthcare team in the medical record
Requirements
  • Bachelor's Degree in Pharmacy; Pharm D preferred
  • One year experience in long-term care
  • Licenses/Certifications: As required by government regulations and business needs
